Healthtech startup Praan Health secured Rs 8.5 crore seed funding led by Rainmatter Investments for chronic care platform development

Bengaluru, India – Healthtech startup Praan Health has raised ₹8.5 crore in seed funding, led by Rainmatter Investments, the investment arm of Zerodha. The fresh capital will be used to develop its AI-enabled chronic care management platform, scale its technology infrastructure, and expand clinical operations across India.
Reimagining Chronic Disease Management
Founded in 2023, Praan Health is building a digital-first chronic care platform focused on helping patients manage long-term conditions such as diabetes, hypertension, cardiovascular disease, and thyroid disorders. The platform combines AI-based health tracking, remote monitoring, and personalized clinical care to enable patients to take control of their health from home.
Praan Health’s integrated care model brings together doctors, nutritionists, and health coaches through a single digital interface. Patients receive continuous monitoring, timely interventions, and actionable insights — making chronic care more proactive and preventive rather than reactive.
According to Praan Health’s co-founder and CEO, the goal is to bridge the gap between lifestyle management and clinical supervision. “Chronic diseases are a growing challenge in India, and we aim to create a technology-driven system that supports patients beyond hospital visits,” he said.

The seed round was led by Rainmatter Investments, which focuses on supporting technology-driven solutions that promote well-being, sustainability, and health innovation.
